Official Enclosure Checklist

Documents Required for Uttar Pradesh Caste Certificate (Full 2026 Checklist)

The definitive document checklist for OBC, SC, and ST applicants in Uttar Pradesh. Avoid rejection by preparing your Self-Declaration Form, Parivar Register Nakal, and photo under 20KB.

Uttar Pradesh Document Verification Ground Rule

In Uttar Pradesh, the Revenue Department requires two non-negotiable documents that differ from other states: the स्वप्रमाणित घोषणा पत्र (Self-Declaration Affidavit) and the परिवार रजिस्टर की नकल (Parivar Register Extract). If either of these is missing or illegible, the Halka Lekhpal will return the file with an objection.

1Self-Declaration Form (स्वप्रमाणित घोषणा पत्र - Mandatory)

Under the UP Janhit Guarantee Act, you do not need an expensive ₹100 stamp paper notary affidavit for applying online. A self-signed declaration on plain paper in the official format is legally sufficient:

How to Prepare:

  1. Download the official Government of UP template: स्वप्रमाणित घोषणा पत्र PDF प्रारूप (Official Download) ↗.
  2. Print the form, fill the applicant's name, father's name, age, village/ward, tehsil, and declared caste/sub-caste.
  3. Sign by hand at the bottom with the date and place. (If applicant is a minor under 18, father or mother must sign as guardian).
  4. Scan in JPG or PDF format, strictly under 100 KB.

2Family Record & Proof of Residence in Uttar Pradesh

The reviewing Lekhpal checks whether your family is an established resident of the village or town:

परिवार रजिस्टर की नकल (Rural)

Issued by the Gram Panchayat Adhikari (Secretary). Shows applicant's name listed under the head of the family.

Ration Card / Nagar Nigam Ward List (Urban)

Food and civil supplies ration card extract (NFSA slip) or Municipal Corporation electoral slip for city residents.

Aadhaar Card (Applicant)

Showing permanent address in Uttar Pradesh and complete date of birth.

Ancestral Land Khatauni / Khasra

Latest certified copy of UP Bhulekh Khatauni registered in father's or grandfather's name.

3Paternal Caste Proof & Local Verification Slips

In Uttar Pradesh, caste is strictly inherited through the biological father. Provide any of the following to substantiate your community:

Option A: Father's Digital Caste Certificate

If your father has an existing caste certificate issued by any Tehsil in Uttar Pradesh, upload a scanned copy. This provides near-instant automatic validation by the Lekhpal.

Option B: Real Brother's or Paternal Uncle's Certificate

If the father does not have a digital certificate, a real brother's caste certificate accompanied by the Parivar Register Nakal proving common paternity is accepted.

Option C: Gram Pradhan / Ward Parshad Certificate (पार्षद / प्रधान प्रमाण पत्र)

If no family member has a previous caste certificate, get an official recommendation on the official letterhead of your Village Pradhan (for rural areas) or Ward Councillor (for urban areas) certifying that the family has belonged to the specified sub-caste for generations.
